Charges will apply for services received. Full payment of fees is expected on date of service, unless other payment arrangements are made and approved (i.e., sliding fee scale).
Proof of income is required for sliding fee scale to be applied and must be provided on the day of service.
The following are acceptable for proof of income:

Confidential Sexually Transmitted Disease (STD) counseling and testing services can be obtained at the Bay County Health Department
for the following: Chlamydia, Gonorrhea, Herpes, Syphilis, Hepatitis B, Hepatitis C, Trichomoniasis and Human Immunodeficiency Virus (HIV)
and other Sexually Transmitted Diseases.
