Form can be filled out on Google then emailed the email address at bottom of form. If you are using Internet Explorer you will need to print form or save it to fill then send in. If you still having issues using the form then please send your request for pet food to bollmanje@baycounty.net or animalservices@baycounty.net make sure to have your name & phone number in the email so we can call you.
