What is Bay Health Plan?
A community-sponsored program for eligible residents between 19 and 64, who do not qualify for any other governmental insurance program. You may qualify for Bay Health Plan if your live in Bay County, Michigan, have no health insurance, and meet income requirements. You must complete an eligibility form at the Bay County Health Department or participating enrollment site.
This is not insurance. Bay Health Plan provides limited services for doctor visits, prescriptions, lab tests and x-rays. Bay Health Plan does not pay for hospital or other inpatient services. If you receive services that are not covered through Bay Health Plan, you will be responsible for payment.
Bay Health Plan Coverage:
- Primary care provider office visits.
- Specialty care office visits.
- Prescription medication (formulary only).
- Outpatient laboratory and radiology services.
- Medical supplies.
