Bay County Community Health Assessment

The Bay County Health Department is currently undertaking a Community Health Assessment with the full partnership of McLaren Bay Region, the Bay Health Plan, and Bay Arenac Behavioral Health under whose support and guidance the assessment will function and who will share responsibility for the success of the assessment.

Goals of the Community Health Assessment

1. To determine the health status of Bay County residents and assess the capacity of the public health infrastructure.

2. To identify gaps in programs and services that are needed within the community.

3. To improve existing programs and services to better serve Bay County residents.

  

Community Health Assessment Structure

1. Phone Survey - Between January 16th and January 27th, residents of Bay county will be randomly selected to participate in a brief telephone survey. The phone survey is completely confidential. Your participation is vital to the success of the health assessment. Please take a few minutes to participate in the survey.

2. Web Survey - An online survey will be available shortly for the public. This survey will consist of similiar questions as the phone survey but will allow for more comment and feedback.

3. Focus Groups - Focus groups will also be conducted to gather information regarding the health status of the community. Focus groups will be conducted in March.

4. Existing Data - Existing data will also be collected and analyzed. Existing data will be collected through morbidity and mortality rates, hospital discharge records, the U.S. Census, and other local, state, or national surveys.

5. Reporting - A report of the health assessment findings will be made available to the public.
