Case Management
Bay County Division on Aging have four Case Managers on Staff;

- Jessica Somerlott, LBSW
Case Managers complete in home visits to evaluate seniors needs. Requests for in home services come from various resources such as hospital social worker staff, medical professionals, home health care agencies or family members and directly from seniors. Case Managers are able to help assist with access to programs such as Home Delivered Meals, Homemaking, Personal Care, Medicaid applications, Medicare questions, and referrals to other agencies and programs as needed. The Case Management team is an advocate for senior resources.
Each Case Manager cover a specific area of Bay County. An intake will be completed and a Case Manager will be assigned, with a in home visit to follow.
