Bay County Division on Aging is dedicated to providing continued opportunities for the community's older adults by maintaining independence, offering social opportunities, and enhancing their lives through quality services in a cost-effective manner.

Case Management

Bay County Division on Aging have four Case Managers on Staff;

Beth Eurich, LBSW
Sue Howard
Jessica Somerlott, LBSW

Case Managers complete in home visits to evaluate seniors needs. Requests for in home services come from various resources such as hospital social worker staff, medical professionals, home health care agencies or family members and directly from seniors. Case Managers are able to help assist with access to programs such as Home Delivered Meals, Homemaking, Personal Care, Medicaid applications, Medicare questions, and referrals to other agencies and programs as needed. The Case Management team is an advocate for senior resources.

Each Case Manager cover a specific area of Bay County. To request services with a Case Manager or any general information contact the Bay County Division on Aging main desk at (989) 895-4100 or toll free 1-877-229-9960. An intake will be completed and a Case Manager will be assigned, with a in home visit to follow.
